We ask that all of our volunteers review the following documents to understand the expectations of being a Millcreek Volunteer:

• Alcohol/Drug-Free Workplace Policy (PDF) 
• Tobacco-Free/Violence-Free Workplace Policy (PDF) 

Once you have reviewed the documents above, please fill out the three online forms below:

• Volunteer Agreement
• Consent, Media Release & Liability Waiver
• Disclosure of Private Business Interests*
• Consent for Sex Offender Background Check **

*This form must be printed and notarized under certain circumstances. 

** Only required if working with youth.
